- Birth certificate (2010 onward) or valid passport
- Proof of address (≤ 60 days). If not in your name: DTOP-DIS-156 + ID
- Current license (valid or expired)
- Form DTOP-DIS-256

- $11 (code 5120) or $35 if expired for >30 days (code 5120)
- $17 (code 2028) if changing to Real ID
- $1 Anatomical Donation Law (code 5120)
